HR will send new hires the below Payroll forms prior to your orientation meeting. You may also print and bring the completed Payroll forms (#'s 1-6 below) to the meeting. A HR representative will review your forms and answer questions at your orientation.

- General Information Sheet
- Voluntary Self-Identification Form
- 2023 Federal W-4 form
- IT 4 Ohio state tax form
- Manual Acknowledgement: Staff or Faculty
Payroll Documents & Information

Full time employees are eligible to enroll in benefits. Benefits are effective on the first day of the month on or after your hire date (i.e if your hire date is on the 1st day of the month, your benefits are effective that day; if your hire date is after the 1st day of the month benefits are effective the 1st day of the following month). Children up to age 26 are eligible for medical, dental, & vision benefits. Spouses are only eligible for medical coverage if they do not have an offer of coverage with their own employer or if they are not working. Spouses are eligible for dental and vision coverage regardless of their employer's offer of coverage.
Prior to your benefits enrollment meeting, review and complete the applicable benefit enrollment forms. Forms and instructions are available in the Benefits folder on the HR Faculty & Staff Teams Channel (log-in required). Additional information for benefits enrollment can be found on the HR OneNote in the Teams Channel. If you are not yet able to access Teams, please contact firstname.lastname@example.org for the applicable forms.
TRAINING & EDUCATION
New employees must complete online tutorials during their first 7 days of employment. Information on these tutorials will be provided at your new hire orientation and are available on the HR OneNote in the HR Faculty & Staff Teams Channel (log-in required).
